I (Parent/Guardian) hereby release and forever discharge the West side Flag Football League, their affiliates, managers, members, agents, volunteers, representatives from any physical, psychological injury that my child may suffer in any WFFL's games, events, or tournaments.

I (Parent/Guardian) grant permission to the West side Flag Football League and its affiliates to use any photographs/videos during any and all league events for any legal use, including and not limited to: publicity, copyright purposes, advertising, and web content. I also understand that no royalty, fee or other compensation shall become payable to me or my child by reason of such use.
